Beförderungsschein
Beförderungsschein is the everyday name for the Muster 4, the doctor's prescription of patient transport; MitaBase and this website call it the transport form.
Three names, one paper: Beförderungsschein, Transportschein and Muster 4 mean the same prescription.
What it must contain and when it needs approval is under Muster 4 and Genehmigungspflicht.
