Muster 4
Muster 4 is the form “Verordnung einer Krankenbeförderung”, with which a doctor prescribes a patient ride or a patient transport; MitaBase and this website call it the transport form.
According to Annex 1 of the Krankentransport-Richtlinie it must state: outbound, return or both, the reason for transport, the treatment day or frequency with the treatment facility, and the medically necessary means of transport.
The prescription should be issued before the ride; only in exceptional cases, above all emergencies, may it be issued afterwards (§ 2 (2) of the directive).
Whether the ride also needs approval is under Genehmigungspflicht; in MitaBase the driver photographs the Muster 4 and the office checks it before submitting; the automatic AI check is in preparation.
